The Family Indemnity Plan covers up to six (6) family members including:

• The Member
• Member’s Spouse
• Children aged 1-25
• Children who are permanently disabled are covered for the duration of their lives once they are enrolled before age 26
• Any two persons chosen from member’s parents and spouse’s parents enrolled before age 76

 

What you need:

• Government issued identification for  applicant and all persons who will be covered under the plan (over the age of 18 years)
• Birth certificates for yourself and the persons you want to insure. Birth certificates need to have complete information
• Documents relating to name change
• First month’s premium
• FIP Enrollment Form
• Proof of Address for the primary certificate holder and all proposed insured age 18 years and over, in the form of a recent utility bill (no more than 3 months), or an account statement from a regulated financial institution
• Marriage certificate or a Declaration of partnership document which can be obtained from a Justice of peace or a Lawyer for insured husband or spouse.

Critical Illness Rider

Coverage for critical illnesses such as Cancer, Heart Attack, Stroke, Paralysis and Major Burns. Up to $30,000 coverage available should you be diagnosed with a critical illness. You can sign up for the rider once you have enrolled in the Family Indemnity Plan.

• No medical required
• Must apply before the age of 60
• 6 month waiting period
• Your family will receive a refund of your critical illness rider premium payments should you pass away without making a critical illness claim before the age of 75
